Description

This form is authorized for use by Offices of Primary Responsibility (OPR) and Forms Management Officers (FMO) to review new or revised digital forms submitted to them for processing by designated forms developers.

All forms must conform to the requirements of FAA Order 1330.1C, FAA Forms Management for compliance to logical form structure and flow, Plain Language, Adobe Acrobat Portable Document Format (PDF), and Accessibility/Section 508. Also, the Paperwork Reduction Act (PRA) and Privacy Act (PA), as applicable. All form files will be tested by the OPR and FMO prior to submission to the FAA Forms Program Office, ASP-110. Refer to Appendix D. of the order for detailed instructions on how to complete this form.
