This form is authorized for use by Offices of Primary Responsibility (OPR) and Forms Management Officers (FMO) to prevent the automatic replenishment of printed form stocks when a revision or other factor affecting the use of a form is proposed or, to cancel the print request in its entirety due to a form cancellation. A separate Forms Stop Order form, FAA 1330-4, is required for each individual form being canceled when it is necessary to purge physical hardcopy forms stocks from distribution points and/or printing jackets of canceled forms. This prevents automatic printing of obsolete forms and avoids unnecessary cost to the agency. Refer to Order 1330.1C, FAA Forms Management, for guidance on this activity. Refer to Appendix B. of the order for detailed instructions on how to complete this form.
